Skilled Nursing Facilities (SNFs) participating in the Medicare program submit annually to CMS cost reports in a standardized electronic format. SNFs file the form CMS-2540-96 in accordance with 42-CFR Part 413.20 and the Provider Reimbursement Manual Part I. Hospital-based SNFs continue to use Form CMS 2552-96 Hospital and Hospital Health Care Complex Cost Report.

The SNF cost reports are a valuable database and provide unique nationwide information for facility characteristics, utilization, cost, charges, reimbursement, financial and income statements.

The Healthcare Cost Report Information System (HCRIS) for SNFs collects annually Facility Cost Report -Data files and stores them in a relational database. The database is continuously updated and is made public quarterly (March, June, September, and December).

Idatareport makes all data available in many formats (Excel, SAS, Stata, etc) ready for download. The data is available by PPS year in the familiar worksheet format for all reporting facilities or customized reports.

HCRIS keeps the highest level of SNF cost report based on its status. From lowest to highest the report status is: as submitted, Settled without audit, Settled with audit, Reopened, Amended.

Many times SNFs submit more than one cost report for a FY with the same status. This can happen if a change in ownership or FY occurs during the year. Idatareport flags the duplicate reports and selects the most representative (e.g., higher number of months) for the analysis. A reporting length indicator is calculated and included with every data set.

This database contains the reports of free-standing facilities. Data from Hospital-based facilities is part of the Hospital Health Care Complex Report (form CMS-2552-96).

Departmental cost to charge ratios (CCRs) for ancillary and routine services plus Per Diem cost for routine services (Used for costing claims databases or benchmarking) are available for download.

Cost Estimation in MEDPAR: Estimate cost in MEDPAR using CCRs from the cost report. We provide the year specific departmental CCRs and the cross-walk to MEDPAR. In addition, we provide the step by step methodology of cost derivation. However, We do not provide any MEDPAR data.

Cost to Charge Ratios for the CMS DRG-recalibration process. CMS to calibrate DRG weights uses cost report data (Fed. Register/ Vol. 75, No. 157/ 08/16/10). Idatareport provides all the steps for this process. Departmental all payer cost and charges from worksheet C part I and Medicare charges from worksheet D-4. Departmental CCRs, CMS aggregated CCRs.

Raw wage index data. Worksheet S-3 (Parts II and III) is the source of data for the Medicare Wage Index. The FY 2011 wage index values are based on FY 2007 cost reports. The FY 20012 will be based on FY 2008 cost reports. Download the wage index raw data for the 2012 wage index update, review your hospital's reporting, find and correct inaccuracies, pre-calculate, or validate the CMS methodology.
